DATA SUBJECT APPLICATION FORM
Pursuant to Article 11 of the Law No. 6698 on the Protection of Personal Data ("KVKK"), individuals defined as data subjects ("Hereinafter referred to as the Applicant") are entitled to submit certain requests regarding the processing of their personal data.
According to the first paragraph of Article 13 of the KVKK, applications to be submitted to our Company, in its capacity as the data controller, must be made in writing or by other methods determined by the Personal Data Protection Board ("Board").
In this context, written applications to our Company may be submitted by printing out this form and:
• Delivering it in person,
• Sending it via notary,
• Signing it with a secure electronic signature, as defined under Law No. 5070 on Electronic Signature, and sending it to the Company's registered email address.
Below are the details regarding how written applications can be delivered to our Company through different channels:
• If submitted in person, the written application must be delivered to the address Mahmutbey Mah. Halkalı Cad. No:37/11 Bağcılar / Istanbul / Türkiye, with the envelope labeled "Request for Information within the Scope of the Personal Data Protection Law."
• If submitted via notary, the written application must be sent to the address Mahmutbey Mah. Halkalı Cad. No:37/11 Bağcılar / Istanbul / Türkiye, with the envelope labeled "Request for Information within the Scope of the Personal Data Protection Law."
• If submitted via Registered Electronic Mail (KEP) signed with a secure electronic signature, the written application must be sent to , with the subject line "Request for Information within the Scope of the Personal Data Protection Law."
Additionally, if other methods are announced by the Board, our Company will provide information on how applications may be submitted through those methods.
Applications submitted to our Company will be responded to within thirty days from the date of receipt, depending on the nature of the request, in accordance with Article 13, paragraph 2 of the KVKK. Responses will be delivered in writing or electronically, in line with the relevant provisions of Article 13 of the KVKK.
